What is Primary Record?
Primary Record is a mobile application that guides individuals and their families in documenting, organizing, and sharing healthcare information.
Founded by two neighbors, Primary Record focuses on the family experience of taking care of each other as they move from point to point in healthcare. With Primary Record, families have all their health information in one place to easily share their health stories with each other and the healthcare team around them.

The Founders
Jean Ross, RN
Co-founderI saw the notebooks and binders families used to keep track of what was happening in critical care. I never questioned why until I started taking 80-year-old adults to medical appointments. No one in healthcare has the complete picture or the time to review the entire history. More and more healthcare relies on families to relay their own health story, yet we have not given them the tools to own their health information in one place.
Jim McIntosh
Co-founderI was a medically complex child (cleft lip and palate in 1973). My parents had to balance work and a child who needed 12 surgeries from 1973 to 1992.
I'm also the father of a special needs child, Mia, who has Down Syndrome. I know first hand what it's like to visit a child in the NICU and learn all they can to be an advocate and educate my family in order to give Mia all the support she needed.
I jumped at the chance to join Jean and lend my 25+ years of software and technology experience to reduce the burden of the disconnected healthcare systems on medically complex family caregivers.
